FASD PRE-SEASON REQUIREMENTS

FASD requires all MS and Varsity players to register with Family ID & obtain a sports physical yearly. You can access FASD’s website for more details as well as registration links by clicking on the box below.

FASD requires all MS and Varsity players to have a concussion baseline test done every 2 years. The test can be taken online through ImPACT and takes about 15 minutes. Click on the link below to access the online test. Use code 48a5965376.

You may also choose to have the test done through your child's doctor or another concussion baseline testing provider. If you choose to go that route, please forward the certificate of completion to fahbregistrar@outlook.com.

USA HOCKEY PRE-SEASON REQUIREMENTS FOR BY 2007 PLAYERS

USA Hockey requires all players who have a BY of 2007 to complete Safe Sport Training prior to the start of the 24-25 season.
